Description

NaphCare is hiring an experienced Medical Discharge Planner just like you to join our team at the Santa Cruz County Jail located in Santa Cruz, CA. Full Time, Part Time, and PRN shifts are available! PRN shifts and hours vary based on coverage needs for FT shifts and will be determined by site leadership and prn employee availability.

Pay rates range from $30.00 - $35.00 per hour based on Shift, FT, PRN, temporary contract, site needs, and experience.

NaphCare has a partnership with NetCE that provides CEU/CME for our staff. NetCE uses a rigorous peer review process to ensure that all activities and content are up-to-date. This service streamlines continuing education for all NaphCare Employees to meet state specific requirements for maintaining licensing.

Position Description:

  • Conduct rounds with physicians to meet with patients to discuss placement orders
  • Assist patients with a specific plan for medication management
  • Include patient in discharge planning
  • Assist patients with the identification of a support network, instructions for future care and follow-up
  • Coordinate care with community agencies, shelters, employment agencies, and drug and alcohol counselors
